Case Manager – Targeted Outreach For Pregnant Women (TOPWA)

Families First is recruiting for the full time position of HIV/AIDS Case Manager for our TOPWA (Targeted Outreach for Pregnant Woman Act) Program. The client base will be pregnant HIV + moms residing in Palm Beach County, including the Glades and Pahokee areas.

SUMMARY:

The case manager will perform comprehensive case management to HIV + pregnant moms in Palm Beach County. This position is responsible for providing integrated and continual care treatment. The case manager must also stay in constant contact with the perinatal services division at Florida Health.  The case manager will connect HIV + pregnant moms to both health and social services, required to maintain stability for the HIV + mom and family.  The goal is to ensure that HIV transmission does not occur between baby and mom during pregnancy or through delivery. The case manager will conduct outreach, home visits, and follow-ups accordingly, prenatally and post, ensuring compliance with medical care and treatment plans. The case manager will then report the findings confidentially and accordingly and is also expected to attend community related HIV/AIDS events.

MINIMUM REQUIREMENTS:

  • Bi-lingual required; specifically Creole/ Tri-lingual preferred (Creole and Spanish)
  • Proficient in the written and oral use of English
  • Able to work flexible hours, including nights and weekends
  • Basic knowledge of HIV/AIDS

  • Knowledge of community resources, and levels of care within the community.
  • The ability to coordinate and navigate services, and follow through to delivery.

EDUCATION & EXPERIENCE: 

  • BA/BS in Social Work or equivalent degree in related field
  • Minimum two to three years related experience

Mental Health Clinician - Belle Glade

Families First of Palm Beach County is seeking a mental health clinician to work with clients through the Behavioral Health Program. This position will be based out of our office in Belle Glade, and the clients served will live predominantly in the western communities of Belle Glade, and Pahokee. 

Must have an ability to conduct in-school based consultations, as well as in-home counseling, and have the ability to work some evenings to accommodate families who work. Experience working with children and families where there is mental health, developmental and/or behavioral concern is extremely helpful. Bilingual English/Spanish or English/Creole is preferred.

Family Support Worker - West Palm Beach Office

Families First of Palm Beach County is seeking a Family Support Worker in the West Palm Beach office. This person will provide intensive family service coordination and home visitation to eligible families. This is a regular nonexempt full-time position, DFWP/EOE, Everify, Level II background screening

Qualifications:
HS diploma or GED required, college degree in related field preferred. Must be bilingual Spanish and English required and Qanjobal language preferred.1 year experience working with children and families required. Ability to work flexible hours a must.

Job Duties and Responsibilities:
Provision and oversight of data entry including maintaining client data base, assures all office work is completed in a timely and professional manner, assures all computer work is completed in a timely and professional manner other data entry and administrative duties performs other related duties as assigned.

To be considered, candidates should:
Have a high school diploma (some college preferred), have knowledge of various computer software programs, be able to operate and have knowledge of computers and other electronic instruments available to the agency, have  extensive knowledge of modern office practices and procedures, business English, spelling, record keeping, knowledge of and sensitivity to service populations cultural and socioeconomic characteristics.
