Project Connect
Project Connect is a collaborative project between Adopt-A-Family of the Palm Beaches and Families First of Palm Beach County to provide community-based care coordination services for families referred to Adopt-A-Family who have multiple issues and needs that impact their ability to maintain stability and self sufficiency.
The Care Coordinator maintains contact with referral sources to assist families in completing their goals towards self-sufficiency. Through the services provided, the long-term outcomes will be achieved as follows:
- Parents will demonstrate progress in debt and financial management.
- Parents will be able to provide for the economic needs of their family.
- Parents will be effective advocates for self and family.
